I hereby grant irrevocable consent to the reproduction and distribution rights of my performance, image, likeness, and voice recorded as a part of Tern Mentoring by Tina Fox Podetize without payment or any other consideration except being invited to appear as a guest.

Any materials submitted, shared or created in connection with this interview and recording become the property of Podetize and it shall have the sole and exclusive right to use, exploit, and distribute all or part of the recordings throughout the world in perpetuity.

I understand that my performance and recordings may be edited, copied, exhibited, published, or distributed by Tina Fox. I understand that this may be used for various content and promotion purposes and that any submitted images or materials and my recordings may be used in part or in their entirety. However, nothing contained in this release form obligates Podetize to use, exploit or distribute any part of the recordings.

I waive the right to inspect or approve the finished product wherein my likeness or voice appears. Additionally, I waive any right to royalties or other compensation arising or related to my participation and performance for the use of my submissions, image, likeness, voice, or recording. By booking and recording an interview, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in a public setting.

By booking and recording an interview, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all liability or claims against any person or organization arising out of the making, producing, reproducing, processing, exhibiting, distributing, publishing, transmitting, or by any means utilizing my performance as a guest, or any submitted images and materials used.
